The nurse will be responsible for completing Start of Care (SOC), Resumption of Care (ROC), Recertification, Skilled Nursing Visits (SNV), OASIS Discharge, and Discharge Summaries. This position is not included in the on-call rotation and does not have a designated number of scheduled hours. The nurse will not function as the primary case manager for patients they admit but will be accountable for accurate and timely completion of all clinical documentation and for effectively communicating pertinent patient information to the assigned case manager and interdisciplinary therapy team. Completes training as required by policy.
