Assists in the collaboration, research, development, implementation, revision and reporting of the resource management program. Collaborating through the multidisciplinary patient care team, ensures coordination of patient care through the development of patient population specific plans of care to address the physical and psychosocial needs in the discharge planning process. Provides assessment and crisis intervention; plans, organizes and implements social work services for patients and families. Provides long-term care planning, patient education and arranges for post-hospitalization services in accordance with the provision of care after discharge. Responsible for complex discharge planning.

Position: PRN (As Needed)

Qualifications

• Graduate of an accredited school with master's degree in Social Work
• Must be licensed in the state of Mississippi (LCSW)
• Clinical background in Medical Social Work. Knowledge of Case Management functions and activities.
• Basic Life Support (BLS) Certification
• Crisis Prevention Intervention (CPI) Certification
