Compassionate Hospice RN Case Manager
UnitedHealth Group
Elite Hospice, part of LHC Group and a member of the UnitedHealth Group family, seeks a Registered Nurse Case Manager to oversee hospice patients’ plans of care and collaborate with the IDG to promote physical, psychosocial, emotional, and spiritual well-being. You will perform initial evaluations, coordinate care, document progress, and educate patients and families on self-care within diverse home settings. A valid RN license and driver’s license are required. #J-18808-Ljbffr
