Responsibilities

• Administer prescribed medications and monitor effects according to care plans
• Provide routine nursing care under supervision of a registered nurse
• Monitor residents’ vital signs and health status; report changes promptly
• Assist with activities of daily living and mobility needs
• Document care provided and maintain accurate charting; complete incident reports as needed
• Collaborate with the interdisciplinary team to coordinate care and ensure resident safety

Requirements

• Active LPN license in New Hampshire or eligibility for licensure
• Experience in long-term care or skilled nursing settings preferred
• Ability to work the night shift (11:00 PM–7:30 AM)
• Strong communication and documentation skills
• Capable of performing duties in a skilled nursing environment and adhering to facility policies
