Physical Therapist Assistant - Home Health
Schedule: Full-time (Flexible)
Location: Chicago, IL
Position Overview
Under the supervision of a licensed Physical Therapist, the Physical Therapist Assistant (PTA) will assist patients in improving mobility, strength, balance, and functional independence in their home environment while supporting positive patient outcomes.
Responsibilities
Implement treatment plans developed by the supervising Physical Therapist.
Assist patients with therapeutic exercises, gait training, and transfer techniques.
Support mobility, balance, and strength-building activities.
Educate patients and caregivers according to the established care plan.
Observe, document, and report patient progress or changes in condition.
Maintain accurate and timely clinical documentation.
Collaborate with Physical Therapists and interdisciplinary care team members.
Follow agency policies, clinical standards, safety protocols, and applicable home health regulations.
Qualifications
Active Illinois Physical Therapist Assistant (PTA) license/certification required.
Graduate of an accredited Physical Therapist Assistant program.
Prior experience working as a Physical Therapist Assistant (PTA) is required.
Home health experience preferred, but not required.
Current CPR certification.
Strong communication, organizational, and time management skills.
Ability to work independently in a home health setting.
Valid driver's license and reliable transportation for travel throughout the Chicago service area.
