Hospice Admissions Registered Nurse

Addus Family Company seeks a compassionate Hospice Admissions RN to coordinate and complete hospice admissions for patients in their homes. In this field-based role, you will conduct initial assessments, verify eligibility, obtain consents, and create individualized plans of care in collaboration with physicians and the interdisciplinary team. You will educate patients and families on hospice philosophy, services, and symptom management, ensuring a smooth transition of care.
