Job Title: Home Health Physical Therapist Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 Weeks Shift: Day Hours per Shift: 8 Hours (08:00 - 16:30) Experience: 1 Year of Skilled Home Health Experience License: Pending License Accepted Certifications: BCLS Required Must-Have: Oasis Experience Description: Home Health Physical Therapist needed for home health services. The position requires 8-hour day shifts. The facility cannot accommodate 4x10s or 3x12s. The expected patient ratio is 1:1. The therapist may visit up to 6 patients per day. The standard productivity points is 30. The traveler is expected to have their own transportation. Driving time and distance will be minimized when scheduling clients. Occasional travel of 50+ miles may be required. The facility requires primary series COVID vaccination and one booster. Oasis experience is required to apply. Home health experience is preferred. The position is open to local candidates within a 50-75 mile radius. Guaranteed hours may vary, with the facility able to call off one shift per pay period. All travelers are expected to float between facilities within the state. Refusal to float may result in termination of assignment and/or forfeiture of guaranteed hours. References are required covering the last year of experience.
