Physical Therapist - Travel Contract

Minimum Requirements

• Master’s or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Bachelor’s degree acceptable if consistent with state requirements.
• Active West Virginia physical therapist license or compact license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience providing home health physical therapy, including creation of plans of care and in-home patient evaluations.
• Ability to travel within assigned coverage areas, including Mason and Jackson counties, and provide patient visits across the geographic region.
• Must meet client-specific professional certification and education requirements indicated by the client.
• Preferred: strong assessment skills and experience documenting progress and outcomes in patient homes.

• Shift Pattern: 5x8 days
• Weekly Hours Guarantee: 40 hours
• Mileage reimbursed between patient visits from first to last visit.
• Coverage Area: Mason County and Jackson County, WV.
• On-call/Weekend Requirement: None
• Primary responsibilities: create plans of care for patients in their homes, provide treatment, and evaluate patient progress.
