RN-Case Manager

Care Manager License: LVN or RN Full time 36 hr per week Major Job Responsibilities Supervise clinical staffs for efficient care delivery in the clinic. Oversee APCM program and coordinate with the clinical staffs. Triage patient calls and facilitate care. Manage Medication Refills as per the Refill policy, and send medication refills per policy. Conduct Nurse visits for patient education and counseling including Medicare Wellness Visit as per the policy. Minimum Qualifications Nursing Degree from an accredited college or university. Current, unencumbered, active RN license in the state. Clinical experience in an acute care setting or long-term care; one year of post-acute care or case manager experience preferred. Previous MDS and admissions experience and working with interdisciplinary team preferred. Supervisory experience is a plus. Skills Injection IM SC skill is required. Blood draws phlebotomy skill is required. Infusion skill is plus. Communication, Organization, Interpersonal and Computer skills. #J-18808-Ljbffr
