Job Description

Job Summary:
Provide specialized outpatient physical therapy services to patients, focusing on restoring mobility, improving function, and reducing pain. Deliver personalized treatment plans based on individual assessments to promote recovery and enhance quality of life.

Responsibilities:

• Evaluate patient conditions and develop customized treatment plans.
• Administer therapeutic exercises, manual therapy, and modalities to facilitate recovery.
• Monitor patient progress and adjust treatment plans accordingly.
• Document clinical assessments, interventions, and progress accurately.
• Educate patients and caregivers on exercises, mobility techniques, and injury prevention strategies.
• Ensure compliance with regulatory standards and facility policies.

Qualifications:

• Hold a valid California Physical Therapist license.
• Possess at least 1 year of outpatient physical therapy experience.
• Demonstrate proficiency in OASIS documentation and home health procedures.
• Be BLS-AHA certified.
• Have strong communication skills and the ability to work independently.
• Own reliable transportation and hold a valid driver's license.
