Job Summary Responsible for creating, leading, and supporting assigned participants in their daily programming in academic, social, life and leisure skills, as well as recreational activities. Assist in the development and implementation of Personal Centered Individual Service Plans (ISP) and ensure compliance with all State, Federal, and third‑party regulatory guidelines. Primary Duties Provide active engagement and support for all individuals assigned, including community integration, training, leisure activities, communication, mobility, behavior management, personal care (e.g., feeding), bus loading/unloading, medication management, and safety and risk management. Assist in the implementation of Person Centered Plans (PCP) outcomes and support activities, ensuring a safe, healthy environment and proper documentation. Maintain accurate PCP documentation, quarterly reports, and daily notes. Maintain a safe, clean, and orderly environment; report unsafe conditions or medical concerns to the Program Manager. Follow established protocols and plans; report any allegations of abuse, neglect, or exploitation; complete incident reports as required. Ensure accurate records and documentation per PCP, regulatory, and company standards; notify the Program Manager of missing or inaccurate information promptly. Communicate with family members, guardians, authorized representatives, residential providers, and support coordinators regarding participant illness, incidents, progress, and other matters. Keep team members apprised of concerns and perform related duties as assigned. Maintain participant records, daily notes, quarterly reports per Medicaid, Licensure, and CARF standards. Adhere to health and safety regulations. Participate in the PCP process (plan, quarterlies, daily notes) and collaborate with the team in implementation, monitoring, and review of the individual’s written plan. Utilize appropriate behavior intervention techniques, proper lifting, feeding, and transferring techniques as written. Complete and pass the Orientation to Development Disabilities (ODD) test and associated competencies. Participate in committee meetings and trainings required by funding sources and the corporation. Additional Duties and Responsibilities Maintain participant records, daily notes, and quarterly reports in compliance with Medicaid, Licensure, and CARF. Ensure adherence to health and safety regulations across all activities. Provide input for PCP processes and collaborate with the team for effective implementation. Utilize behavior intervention, lifting, feeding, and transferring techniques as prescribed. Pass the ODD test and complete all required competencies. Engage in department, participant, and staff committee meetings. Complete mandatory trainings as required by funding sources and the organization. Qualifications High School diploma or GED required. Minimum one (1) year of experience working with adults with developmental disabilities or a related field. Preferred: Combination of college‑level courses in human services and experience with adults with developmental disabilities, or a bachelor’s degree in human services with relevant experience. Knowledge, Skills and Abilities Strong interpersonal and organizational skills; able to relate to people at all levels. Good judgment and decision‑making abilities. Ability to develop and implement professionally written PCPs and supporting documentation. Ability to lead engaging, age‑appropriate activities and social groups. Proficiency in writing routine reports and correspondence. Effective spoken communication skills. Knowledge of communication resources and strategies. Physical ability to support individuals with personal care (feeding, toileting, transferring). For driving duties: current, valid driver’s license; eligibility under ServiceSource driver’s insurance; completion of online driving training. Physical Demands Regular standing, reaching with hands and arms, and verbal communication are required. Occasionally walking, sitting, and lifting or moving items up to 50 pounds.
