Clinical Manager

Gulfstream Staffing has partnered with a medicare-certified Home Health organization that is experiencing a rapid growth phase. We are currently seeking candidates for a full-time Clinical Manager opportunity in Vero Beach.
The Clinical Manager will primarily work in office and will also perform field case management with responsibilities for assessment, coordination of care with other disciplines and communication with referral sources. Coordinate patient admission with other members of the care team. Must have strong case management experience and working knowledge of Medicare. Position : Clinical Manager Location : Vero Beach, FL (Martin/St. Lucie/Indian River Counties). Clincal Manager Duties:- Perform case management, oversight and coordination of care for all skilled disciplines.
- Close communication with social workers
- Coordinate patient admission with other members of the health care team.
- Provide nursing care and treatment of home care patients in accordance with the plan of treatment and approved authorizations. RN will supervise all non-skilled caregivers.
Clinical Manager Qualifications:
- Current RN Florida Professional license
- Minimum of one year experience with Medicare Home Health Case Management
- Experience with Kinnser / WellSky a plus
