Registered Nurse Case Manager

Summary The Case Management Registered Nurse (CM RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. The Social Work and Nursing Case Management Service Line is a multidisciplinary team that addresses the healthcare needs of veterans. The incumbent will provide nursing support to inpatient medicine and specialty care teams, aiding in the coordination of post–discharge services. Qualifications English Language Proficiency Proficient in basic written and spoken English, as per 38 U.S.C. 7403(f) Graduate of a school of professional nursing approved by the appropriate accrediting agency (ACEN or CCNE) Or individuals attending a master’s‑level bridge program may become registered as a nurse with a state licensing board prior to completion of the bridge program Upon achievement of a state license, the individual may be appointed on a temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program In case of graduates of foreign schools, possession of a current, full, active, and unrestricted registration will meet the requirement for appointment as a nurse Credit for foreign nursing education higher than an associate degree requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD) Current, full, active, and unrestricted registration as a graduate professional nurse in a U.S. state, territory or commonwealth, or the District of Columbia Graduate Nurse Technician (GNT) Exception: candidates who otherwise meet basic education requirements but lack licensure may be appointed at the entry step on a temporary appointment while actively pursuing licensure (may be extended up to two years on a case‑by‑case basis) Grade determinations based on scope, education and dimension criteria (Practice, Veteran/Patient Driven Care, Leadership, Professional Development, Evidence‑Based Practice/Research) Preferred experience: inpatient nursing (medical/surgical/ICU), case management experience, and home health coordination Physical requirements per VA Directive and Handbook 5019 Duties Provides nursing support to inpatient medicine and specialty teams, including assisting with discharges from outside hospitals Provides nursing support to PACT teams, specialty clinics, and community‑based outpatient clinics Supports and leads in the provision of appropriate care for veterans ranging from 18 through geriatric years Evaluates care and outcomes to ensure timely and appropriate provision of services Influences care outcomes by collaborating with members of the interdisciplinary team and executing responsibilities that demonstrate leadership, experience, and creative approaches to the management of complex client care Coordinates care with multiple providers across all levels and sites of care Addresses psychosocial, nursing, and medical needs of patients and their families/caregivers through participation in interdisciplinary patient care management practice Benefits Competitive salary with regular increases and potential for performance awards 50 days of paid time off per year (26 days annual leave, 13 days sick leave, 11 paid Federal holidays) Traditional federal pension (5 years vesting) and federal 401(k) with up to 5% contributions by VA Federal health, vision, dental, term life, and long‑term care insurance (many federal insurance programs can be carried into retirement) Full, unrestricted license from any U.S. state or territory required Full‑time schedule (Tuesday‑Saturday, 8:00 am‑4:30 pm); no telework or virtual option Relocation and recruitment incentives: Not authorized #J-18808-Ljbffr
