Field Care Navigator: LTSS & Waiver Services
Molina Healthcare of Illinois
Molina Healthcare is seeking an experienced Care Manager to support Medicaid members in Northern Cook County and surrounding areas. The role includes face‑to‑face home visits, coordinating LTSS and behavioral health services, and working with a multidisciplinary team to improve member outcomes. Preferred candidates have 2+ years in healthcare, care management experience, and a valid driver’s license. Mileage is reimbursed; hours are Mon–Fri, 8 AM–5 PM CST.
