LPN Case Manager

The Fort Mojave Indian Tribe

The Fort Mojave Tribe is searching for a Licensed Practical Nurse for our Special Diabetes Program . This position provides out-patient nursing care, coordination, evaluation, and education services to diabetic/pre-diabetic members of the Fort Mojave Tribal community and their families. This position includes planning and implementing community-based diabetes prevention activities. This newly created position will assist with ensuring program goals are delivered in a timely manner and in compliance with all agencies involved.

This position includes providing home visits to patients, education and staff supervision, as needed for our Community Health Representatives. 

Essential Duties & Responsibilities:

  • Collaborates with Community Health Representatives and Community Health nurse in the provision of care in the community.
  • Make home visits as needed for assessment, case management, treatment planning and educational purposes.
  • Assists with the oversight of the case management of the SDPI program with direct management of care delegated to Community Health Nurse.
  • Leads staff, case management and CHR meetings to review and assess progress with community-based care issues specific to each patient and patient group.
  • Develop, coordinate and implement frequent educational programs and educational material for patient groups within the community. These programs shall include, but not be limited to diabetes prevention, fitness, control of disease complications, general health, elder health, prenatal and newborn care and health.
  • Organizes and documents appropriate training opportunities for outreach staff.
  • Participates in data collection and documentation related to activities conducted for the Special Diabetes Program. Assists in gathering data relating to the QA/QM program for the facility.
  • Develop and maintain cooperative working relationships with agencies and organizations interested in public health care.
  • Collaborate with health specialists and community groups to determine community health needs and the availability of services and to develop goals for meeting needs.

Provide guidance to agencies and organizations on assessment of health education needs and on development and delivery of health education programs.

Qualifications:

  • Required
  • Licensed Practical Nurse in good standing with current licensure in one of the 50 states in the US.
  • At least 2 years of experience as an LPN.
  • Current BLS.
  • Must have current driver license.
