Cross Country Nurses is seeking a travel nurse RN Home Health for a travel nursing job in Modesto, California. This position involves delivering holistic and individualized in-home nursing care, coordinating interdisciplinary care plans, and maintaining collaborative relationships with patients, families, and healthcare providers. Job Details Position Description: Perform routine in-home nursing assignments with accountability for patient assessment, care delivery, education, and care transitions. Coordinate interdisciplinary plans of care encompassing psychological, sociocultural, spiritual, economic, and lifestyle factors. Required Qualifications: Valid RN license in CA, BLS certification, minimum of 2 years of experience providing in-home or community nursing care, proficiency with Epic and blood/blood products management, experience with adult home health, administering medications and IV therapy, and charting experience with Epic. Schedule: Day shift, 8 hours per day, minimum of 40 hours per week. Additional Information: Involves routine patient visits, on-call availability one day per month, and float to counties including Stanislaus, San Joaquin, and Merced. Must complete required onboarding modules, provide verification of work history over the past 7 years, and adhere to holiday RTO policies.
