### TITLE
Physical Therapist

### SCHEDULE
Shift: 8 hours

### ASSIGNMENT DETAILS
Contract Length: 13 weeks
Guaranteed Hours: 40

### REQUIREMENTS
- Current State License as a Physical Therapist - Required
- Proof of current CPR certification - Required
- Current driver's license. Must have reliable transportation and insurance - Required
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
- Previous home care experience - Preferred
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required

### ROLES & RESPONSIBILITIES
- Conduct clinical assessments to identify patient needs and determine the appropriateness of patient care.
- Document and review patient care plans, including progress notes and other clinical records; revise plans based on ongoing assessments.
- Develop and implement treatment plans for patients with neuromuscular, neurological, and orthopedic disorders.
- Assist and educate patients and families on safe transfer techniques and the proper use of assistive devices.
- Provide patient education regarding therapeutic procedures tailored to patient needs and readiness.
- Collaborate with interdisciplinary team members to ensure comprehensive patient care.
