How you’ll make an impact in this role Deliver direct patient care as a Licensed Diabetes Educator, improving patient outcomes through personalized nutrition therapy and evidence-based diabetes self-management education. Educate patients, families, and healthcare staff on diabetes management, strengthening patient care adherence and reducing preventable complications through practical nutritional guidance. Integrate medical nutrition therapy into interdisciplinary team plans, enhancing care coordination and supporting measurable quality improvement in diabetes outcomes. Develop and monitor individualized education plans, ensuring clinical documentation in Electronic Health Records supports continuity of care and performance reporting. What minimum requirements you’ll need Licensure / Certification / Registration: Dietitian/Nutritionist credentialed from the Commission on Dietetic Registration obtained prior to hire date or job transfer date required. Educator specializing in Diabetes Management preferred. Education: High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
