Physical Therapist - Home Health

Assignment Duration

This is a 13-week contract position.

Schedule

Day shift, Monday to Friday, from 8 AM to 5 PM. There is no weekend work required.

Job Description

The Physical Therapist will provide home health services to patients within a 60-mile radius. The role includes evaluating patient needs and developing appropriate treatment plans to facilitate recovery and rehabilitation.

Responsibilities

• Conduct patient evaluations and assessments.
• Implement and adjust individualized treatment plans.
• Monitor and document patient progress in the EMR system (Meditech).
• Collaborate with an interdisciplinary team for enhanced patient care.
• Ensure compliance with healthcare regulations and standards.

Required Experience / Certifications / Licensure

• At least 1 year of experience in physical therapy, preferably in a home health setting.
• Current Missouri Physical Therapist State license (PT compact privileges possible).
• BLS certification required.
