Responsibilities Service Planning: Create individualized service plans that reflect each person's needs and goals Resource Coordination: Connect individuals with providers, community supports, and natural supports that fit their needs Monitoring Supports: Check that services are delivered properly and match what is outlined in the service plan Health And Safety: Promote well-being by addressing safety concerns and supporting healthy choices Person Centered Support: Encourage individuality, choice, and control in the lives of the people you assist Work Schedule and Additional Information Full-time employment. Work hours are 8:30 AM to 4:30 PM, Monday – Friday, with a 30‑minute lunch break. Telework: you will not have the option to telework in this position. Qualifications Two years of experience as a County Social Services Aide 3 and two years of college‑level coursework including 12 credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or A bachelor’s degree supplemented by 12 credits in the social sciences mentioned above; or Any equivalent combination of experience and training that includes 12 credits in the social sciences and one year of experience as a County Social Services Aide 3 or in a similar paraprofessional case management position. Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree or credits. Other Requirements PA residency requirement is currently waived for this title. Valid driver’s license, not suspended. Must be able to perform essential job functions.
