Job descriptionRN - Registered Nurse Case ManagerJob SummaryA qualified Registered Nurse candidate to perform the responsibilities of case management of home health care services to our patients based on a physician's plan of care.Responsibilities and DutiesCoordinate multi disciplinary activities related to the patient's care planManage home health care personnelProcess and manage authorizations for patient care servicesDocument and chart data using electronic medical record systemComplete timely documentation according to the rules of participation in medical service plansQualifications and SkillsOne or more years experience in medical-surgical adult health nursingExperience in home health preferred but not requiredMust be experienced in managing care plansExperience with coordination of multi disciplinary activities and management of home health care personnelSelf-starter with excellent organizational and writing skillsExperience in current Medicaid, Medicare Regulations and Joint Commission standards home health standards helpfulHas maintained a current and active registered nurse license in the State of LouisianaExperienced in electronic documentationBenefitsPremium Salary position with additional compensation:Mileage ReimbursementRetirement PlanOn-Call PayNew Admit compensationMedical benefits - employer participation in costsDental Vision Life InsurancePaid time off after 3 months of full time employmentHoliday PayPaid Vacation after one yearJob Types: Part-time, PRN Schedule: Education:Associates or Bachelor's DegreeExperience: med surg: 1 year (Required)
