Discharge & Care Coordination Case Manager
Case Management Society of America Inc
Case Management Society of America (CMSA) ® in Camden, New Jersey, is seeking a dedicated professional to provide psychosocial assessments and facilitate complex discharge planning for patients and their families. The ideal candidate will have experience in health care and must hold a master's degree in social work or a valid RN license. This role involves coordinating care authorization processes, managing referrals to care agencies, and working closely with families to ensure successful discharge plans are formulated. Candidates should have a minimum of 3-5 years of relevant experience. Licenses such as NJ SW LCSW or RN are preferred. #J-18808-Ljbffr
