Responsibilities Ensure that all activities performed align with the vision of Elara Caring’s board, executive team, and Home Health leadership. Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule. Assess assigned caseload of home health care patients and families to identify physical, psychosocial, and environmental needs through documentation, clinical records, interdisciplinary reports, and on‑site evaluations. Assume primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment. Assess patient needs and obtain data on physical, psychological, social, and spiritual factors influencing health status, incorporating that data into the plan of care. Make initial evaluations and re‑evaluate nursing needs during each visit. Make follow‑up visits according to patient, family, and facility needs. Communicate significant findings, problems, and changes in health condition or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other personnel involved with patient care. Revise the plan of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility, and other team members. Qualifications Current, unrestricted state RN license. 1+ year experience in a clinical care setting as a nurse. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment. Ability and willingness to travel within branch/office coverage area (50% of time). Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws. Associate’s or Bachelor’s Degree in Nursing preferred. Previous Home Health or Hospice experience preferred. Experience with HomeCare HomeBase preferred.
