Job Opportunity

Position Overview

This is an exciting opportunity for a qualified Care Manager with experience in Care Coordination and Discharge Planning at an acute care facility. The position is available for immediate start and requires a commitment to providing quality patient care.

Key Responsibilities

• Perform Care Coordination and Discharge Planning for up to 25 patients daily.
• Assess patients' needs and develop appropriate Care Plans in accordance with established guidelines.
• Utilize computerized documentation systems for efficient record-keeping.
• Collaborate with interdisciplinary teams to support patient care and ensure discharge processes are seamless.
• Manage patient admissions, determine medical necessity, and adhere to HIPAA guidelines.
• Engage in disease management and coordinate with facilities regarding home health services and Durable Medical Equipment (DME) needs.

Qualifications

• Minimum of 2 years of acute medical Care Manager experience with a focus on Care Coordination and Discharge Planning.
• Valid CA state license required (pending licenses accepted if licensed upon start date).
• Experience with charting systems, specifically Epic, is required.
• Knowledge of CMS guidelines and CPT coding is preferred.
• Previous charge experience and familiarity with patient admission criteria are advantageous.
• Must demonstrate strong skills in patient assessment and care management.

Work Schedule

• Days: Monday to Friday, 8:00 AM to 4:30 PM
• Weekend Requirement: Typically one full weekend every four weeks, subject to departmental needs.
• Must be available to work three of the following holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day.
