Allied Technician - Physical Therapist

Physical Therapist (PT) - Home HealthWe are seeking a dedicated and experienced Physical Therapist to join our Home Health team. The ideal candidate will have a minimum of one year of experience in home health care and be proficient in OASIS documentation.Job ResponsibilitiesPerform patient assessments and develop individualized treatment plans.Conduct regular evaluations to track patient progress and adjust treatment strategies as needed.Provide therapeutic interventions to enhance patient mobility, strength, and overall functional abilities.Educate patients and caregivers on exercises and strategies for home-based rehabilitation.Complete OASIS documentation and ensure compliance with agency policies.Manage scheduling and coordinate with other healthcare professionals.Monitor vital signs and assess pain, skin integrity, and overall health status.QualificationsMust hold a current Physical Therapist license in Virginia.One year of experience in home health care required.Proficiency in OASIS documentation is essential.BLS certification is required; Manual Therapy certification is preferred.Ability to manage time effectively and document patient visits accurately.SkillsAssessment skills and pain assessment capabilities.Ability to identify environmental barriers and provide solutions.Knowledge of adaptive devices, orthopedics, and various patient populations.Experience in managing conditions such as COPD, CHF, and neurologic disorders.Competence in using equipment such as gait belts, Hoyer lifts, and specialty beds.Work EnvironmentThis position will primarily involve making home visits, with an expected workload of approximately 5 visits per day.
