Responsibilities: Oversee and coordinate daily clinical operations to ensure high-quality patient care and regulatory compliance Supervise, mentor, and evaluate clinical staff including nurses, therapists, and home health aides Conduct patient assessments, care plan development, and ongoing case management Ensure adherence to state, federal, and accreditation standards for home health care Collaborate with physicians, interdisciplinary teams, and families to optimize patient outcomes Monitor clinical documentation for accuracy, completeness, and compliance Identify and implement quality improvement initiatives to enhance care delivery Requirements: Active Registered Nurse (RN) license in the state of Illinois required Minimum 2–3 years of clinical experience in home health, community health, or a related setting Previous supervisory or management experience strongly preferred Strong knowledge of Medicare/Medicaid home health regulations and OASIS documentation Excellent leadership, communication, and organizational skills Proficiency with electronic health record (EHR) systems Compassionate, patient-focused approach with a commitment to quality care About Us: Anuco Home Health Care LLC is a trusted provider of home health services in the Chicago area, dedicated to delivering compassionate, patient-centered care to individuals in the comfort of their own homes.
