Job Description
The Licensed Practical Nurse (LPN) will provide direct, compassionate nursing care to patients in an acute rehabilitation setting under the direction of an RN and/or physician. Responsibilities include monitoring patients' conditions and vital signs, administering medications and treatments as ordered, assisting with activities of daily living, documenting patient care, identifying changes in patient condition, and promptly communicating concerns to the nursing and healthcare team. The LPN will also support patient recovery, maintain a safe and clean care environment, follow infection-control procedures, and work collaboratively with other healthcare professionals to deliver quality patient care.
Education & License- Graduate of an accredited Practical Nursing/LPN program
- Active, unencumbered LPN license appropriate for the facility/state
- Current BLS/CPR certification with hands-on skills verification
- Minimum 1 year of recent acute-care experience in a Hospital, LTAC, or Inpatient Rehabilitation Facility (IRF)
- Experience providing direct patient care in an acute or rehabilitation setting is preferred.
- Ability to work effectively with RNs, physicians, therapists, and other members of the healthcare team.
