Williams Circle Suite 3000 Tucson, AZ 85711, USA Overall Purpose: The Provider Operations Coordinator (POC) role combines operational support initiatives with special projects focused on supporting HEDIS quality gap closures, Risk Adjustment activities, and provider engagement. The position requires daily problem-solving, meticulous tracking, and coordination across departments to meet changing requirements. The POC supervises assigned affiliate groups, provides support to clinics, and engages stakeholders and patients for education and preventive care. Roles and Responsibilities: Build and maintain relationships with assigned affiliate groups and serve as liaison between affiliate groups and internal departments. Advise and support affiliate practices on quality gaps and documentation; review data to confirm gaps. Prioritize patient outreach, assist practices with outreach, and coordinate scheduling of patient visits. Partner with groups and other departments to manage high/rising risk patients and coordinate care. Assist in meeting preparation and participation as requested. Maintain data entry, documentation, and tracking for quality initiatives. Educate patients and affiliates on preventive screenings and their purpose. Perform in‑home HEDIS gap closures for select measures, depending on experience. Other duties as assigned. Education and Experience: High school diploma/GED required; associate degree or equivalent experience preferred. 2+ years experience in healthcare‑related fields required; managed care or health plan experience strongly preferred. Experience with Microsoft Office suite and general office equipment mandatory. Experience with Electronic Health Records mandatory. Medical Assistant Certification or experience as Pharmacy Tech, Radiology Tech, Lab Tech, or similar strongly preferred. Experience with data mining preferred. Medical terminology knowledge mandatory; experience with CPT II codes preferred. Current CPR certification preferred. Work Conditions: Must be able to travel within the assigned region and work up to 90% of the schedule at provider offices. Must have a valid driver’s license, safe driving record, and reliable transportation. Physical Requirements: Work is performed in a typical office environment; seated, talking on the phone, occasional standing or walking, and lifting up to 25 pounds.
