Role Description This is a full-time, on-site Registered Nurse role based in Manatí. The Registered Nurse will provide direct, in-home hospice care, including assessing patient conditions, administering medications, and performing nursing procedures in accordance with established care plans. Day-to-day responsibilities include monitoring symptoms, documenting patient status, coordinating with interdisciplinary team members, and communicating with families about care needs and changes. The nurse will educate patients and caregivers on symptom management, safety, and comfort measures, while following hospice protocols and regulatory requirements. The role also involves participating in care plan meetings, supporting emotional needs of patients and families, and ensuring high-quality, culturally sensitive care. Qualifications Current Registered Nurse (RN) license in Puerto Rico, BSN and eligibility to practice in hospice settings. Clinical skills in pain and symptom management, medication administration, and basic nursing procedures for terminally ill patients. Strong communication and interpersonal abilities to interact effectively with patients, families, and interdisciplinary team members. Organizational and time-management skills to manage home visits, documentation, and coordination of care. Ability to provide empathetic, patient-centered care and support emotional and psychosocial needs at end of life. Experience in hospice, palliative care, home health, or related clinical settings is highly beneficial. Knowledge of relevant healthcare regulations, infection control standards, and patient safety practices. Valid driver’s license, reliable transportation for home visits, and willingness to travel within the North and Metropolitan area. Basic proficiency in electronic health records (EHR).
