MDS Coordinator (Part-Time) — $45–$55/hr

We're looking for a part-time MDS Coordinator to join our team — someone who takes pride in accuracy, knows the RAI process inside and out, and can keep assessments on schedule without breaking a sweat. If "PPS schedule" and "care plan" are second nature to you, keep reading.

What You'll Be Doing

• Completing and coordinating the MDS (Minimum Data Set) assessment process from start to finish

• Ensuring timely and accurate completion of MDS assessments, care plans, and CAAs in line with RAI guidelines

• Tracking PPS and OBRA schedules so nothing slips through the cracks

• Collaborating with nursing, therapy, social services, and dietary teams to capture a complete, accurate picture of each resident

• Reviewing documentation to support appropriate reimbursement and regulatory compliance

• Serving as a go-to resource for questions about resident assessment and care planning

What You Bring

• Current RN or LPN/LVN license in good standing

• Experience with MDS assessments and the RAI process (skilled nursing facility experience preferred)

• Strong attention to detail — you catch what others miss

• Solid understanding of Medicare/Medicaid regulations and reimbursement processes

• Great communication skills and the ability to work cross-functionally with a care team

• Comfort with EHR/MDS software systems
