Physical Therapist (Home Health) in San Ramon, California. This position involves providing outpatient physical therapy services within designated service areas. Candidates are required to bring their California Physical Therapist license on the first day of assignment. The role emphasizes patient-centered care and adherence to facility protocols.
Job Details:
- Location: San Ramon, California
- Schedule: Days shifts, 12 hours per shift, minimum of 36 hours per week
- Required Qualifications: Current California Physical Therapist (PT) license, BLS certification, valid drivers license, auto insurance, auto registration, annual TB screening, MMR, Varicella, Hepatitis B, Tetanus-Diphtheria-Acellular Pertussis (TDaP) (10 years), annual influenza vaccination, drug screen, and physical assessment.
- Additional Information: Additional travel outside the assigned areas may be required. Weekend coverage may be necessary.
- Position Description: The role involves providing outpatient physical therapy services, potentially across multiple locations within designated territories. Candidate must be willing to travel and work weekends if needed, and adhere to all clinical protocols and documentation requirements.
