Assesses and evaluates patient care needs and applies critical thinking skills in patient management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care, including teaching, rounding, coaching, and post-hospital care planning while delegating to others as appropriate. Educates patients and caregivers regarding their plan of care, transition of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, families, and all members of the healthcare team. Manages assignments within the Care Delivery Model while demonstrating effective allocation of material resources. Models the tenets of Relationship Centered Care through actions such as admission welcomes, bedside shift reports, and meaningful rounding on patients. Considers patient values, preferences, and cultural diversity in all aspects of care. Establishes and maintains a therapeutic relationship with patients and families. Uses evidence-based practices to understand patients' perceptions of care. Initiates service recovery for patients and family members with concerns, escalating when necessary. Demonstrates caring, respect, compassion, empathy, and active listening through communication. Supports staff in using evidence-based practices to better understand patients' perceptions of care. Conducts purposeful hourly rounding on assigned patients. Practices safe and efficient patient-centered care, identifying barriers and opportunities for improvement. Demonstrates clinical skills crucial to quality and safety in the patient population served. Implements process improvement strategies based on evidence and institutional procedures. Engages in peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects and nursing research studies. Contributes to quality and safety practices and compliance measures. Implements principles of high reliability to provide a safe environment for patients. Escalates potential safety hazards or gaps from best practices following established protocols. Utilizes information technology by documenting nursing practice to support quality and performance improvement initiatives. Performs charge nurse duties as assigned. Coordinates with management to ensure supplies are utilized appropriately and inventory is sufficient for patient care. Escalates resource deficits to appropriate management. Demonstrates patient-focused and cost-effective approaches to patient care. Documents in an accurate and thorough manner in compliance with legal requirements and standards of care. Facilitates patient throughput on a shift basis. Contributes to a supportive environment for clinical colleagues, patients, and families. Advances clinical competence in nursing practice. Projects a professional image to colleagues and communicates with respect and empathy. Incorporates ethical principles into decision-making for patients and families. Encourages a spirit of lifelong learning and innovation. Acquires knowledge relevant to the role and patient population.
