Job Description
Title: Pediatric Case Manager RN

Schedule:
- Shift: 5x8 - Day shift (start times: 07:00-15:30, 07:30-16:00, 08:00-16:30, or 08:30-17:00)
- Weekend requirement: Yes - every other weekend

Assignment Details:
- Guaranteed hours: 72 hours per 2 weeks

Requirements:
- Must have Alaska RN license
- Minimum 2 years nursing experience
- Pediatric/NICU experience required
- Acute care hospital experience required
- Epic charting experience required

Roles & Responsibilities:
- Manage pediatric cases, focusing on neonates and infants.
- Collaborate with interdisciplinary teams including IV teams, PT, RT, social services, and pharmacy.
- Facilitate discharge planning and ensure continuity of care.
- Conduct assessments and develop care plans tailored to patient needs.
- Provide education and support to families regarding care transitions.

Compliance / Notes:
- Orientation: 24 hours
- Only 1 time-off request allowed per 13-week assignment (5 days or less); no holiday requests for auto-offer
- Must live >50 miles from facility
- Willingness to work 2 of the 3 major holidays: Thanksgiving, Christmas, New Year
