This role involves delivering quality therapy services to patients through evaluations, developing individualized treatment plans, and actively participating in discharge planning. Services are provided within an interdisciplinary team framework, targeting skilled nursing patients with an average length of stay of approximately 14 days. Job Details: Position Description: The physical therapist delivers therapy services to patients, performs evaluations, and implements treatment plans to maximize functional outcomes. Responsibilities include participating in discharge planning and reassessment of treatment results, delivering services within an interdisciplinary team approach, and maintaining compliance with state licensure and organizational standards. Certifications and Education Requirements: Valid Texas and national license; BLS Certification. Schedule: Monday through Friday, 7:00 AM to 4:00 PM or 8:00 AM to 5:00 PM, with no weekends or call responsibilities. Required Qualifications: Texas and national license; BLS certification. Additional Information: The position involves working in a 25-bed critical access hospital, treating skilled nursing patients with various conditions, including fractures, orthopedic injuries, neuro cases, and amputations. The facility supports a collaborative therapy team, with opportunities for shadowing and onsite education during onboarding. Candidates may be asked to rotate in on holidays, and scrub color is maroon. The assignment duration is from 09/07/2026 to 11/30/2026, with a minimum of 40 hours per week.
