Compassionate Hospice RN Case Manager

Heart of Hospice, part of LHC Group, is seeking an experienced Registered Nurse Case Manager to deliver and coordinate the Plan of Care for hospice patients. You will assess needs, collaborate with the care team, and ensure compassionate, quality end-of-life care in patients' homes. This Mississippi role requires an active RN license in Mississippi, a valid driver's license with reliable transportation, and typically 1+ year of RN experience in a healthcare setting; CPR certification is #J-18808-Ljbffr UnitedHealth Group
