Clinical Liaison (LPN) | Home Health & Hospice

Are you an LPN with home health experience who's ready to take your clinical know-how to the hospital bedside? Interim HealthCare of Oklahoma City is hiring a Clinical Liaison to meet patients before discharge, educate families on home health and hospice, and make sure every patient's transition from hospital to home goes smoothly. You'll set your own day, own your relationships, and be the first friendly face patients see from our team. Your schedule

• Monday through Friday, daytime hours
• Travel between hospitals, rehab facilities, and skilled nursing facilities across the OKC metro

What you'll do as a Clinical Liaison

• Visit patients at the bedside in hospitals, rehab centers, and skilled nursing facilities to explain home health and hospice services in a way that's easy to understand
• Review clinical records and gather the information our clinical team needs to confirm a patient is appropriate for home health or hospice
• Partner with discharge planners, case managers, social workers, and physicians so every referral is complete and accurate
• Coordinate with our intake team so patients start care at home on time
• Answer questions from patients and families and help them feel confident about what comes after discharge
• Build and maintain relationships with referral sources across the OKC metro
• Keep accurate, HIPAA-compliant records of referral activity

What we're looking for

• Active Oklahoma LPN license
• At least [2+] years of LPN experience
• Previous home health experience is required. Clinical field experience counts; you don't need prior liaison experience.
• Liaison, hospital, or hospice experience is a plus
• Established relationships with OKC-area hospitals, case managers, or discharge planners are a bonus
• Working knowledge of Medicare home health and hospice eligibility criteria
• A confident, warm communicator who is comfortable at the bedside and in front of referral sources
• Valid driver's license, reliable transportation, and current auto insurance
• Current CPR certification
