Job Details: Job Title: RN (Registered Nurse) Location: West Bronx, NY Setting: Hospital Pay Range: 2492.48 - 2617.10 Schedule: 8:00 AM - 4:30 PM Contract Length: 13 weeks Candidate Type: Traveler Requirements: Must have 1 year of inpatient case management experience Responsibilities: Provide discharge planning and referrals for inpatient hospital patients. Collaborate with interdisciplinary teams, including physicians and social workers. Manage a caseload of at least 20 patients, including discharge to home, community, or SNF. Obtain housing, shelter packets, and specialized referrals such as Hemodialysis and Wound Vac. Utilize electronic medical records (Allscripts) to document patient progress and discharge plans.
