Family Nurse Practitioner or Physician Assistant - Primary Care

Community Care Physicians seeks a Family Medicine NP/PA for our Slingerlands practice. In this outpatient role, you’ll provide comprehensive primary care across the lifespan, including acute visits, chronic disease management, preventive care, and patient education.
