Summary Position Type: Non-exempt, full-time or part-time, hourly role. Scope of Role: Responsible for providing direct support within a single program or unit, typically supporting 4‑6 individuals in residential settings or up to 10 individuals in periodic programs. Key Responsibilities: Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, and skill/behavioral development. Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living. Services And Supports Direct Support: Provide assistance in accordance with individual service or program plans; support socialization, behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other daily living activities. Records: Accurately document progress and activity; review logs to stay abreast of changes in service plans; maintain confidentiality. Management of Individuals’ Assets: Make cost‑effective and appropriate purchases within the individual’s budget and based on their choice or preference. Relationships: Maintain healthy and professional relationships with individuals, friends, families, guardians, case managers; greet family members and other visitors; implement Sevita Customer Service Standards. Reporting: Report any instance of alleged abuse or neglect according to internal and external standards; report medical, behavioral, and other incidents in accordance with Sevita policy and external requirements. Rights: Maintain confidentiality, respect the rights of persons being served per the bill of rights; practice universal precautions; assist individuals in exercising their rights. Health Care Appointments: Accompany individuals to medical appointments as needed; relay orders and information to and from medical providers. Medications: Accurately administer or support self‑administration of medication; document delivery; promptly report errors; maintain security of controlled medications and supplies. General Health Care: Monitor individuals’ health; document concerns; communicate with nurse or supervisor as appropriate; call 911 in emergencies; follow individual health care directives. Medical Supplies and Equipment: Follow procedures for utilizing medical equipment; report malfunction or disrepair. Meal Preparation: Prepare or support individuals in preparing food per planned menus and individual choice; know special diets and prepare accordingly; monitor mealtime and implement dining plans. Employment Responsibilities Training: Attend orientation and ongoing training; participate in monthly staff meetings. Employee Scheduling: Work scheduled hours; arrive on time; remain on shift until replacement arrives; notify supervisor in advance when unable to work as scheduled; may fill in for other shifts when required without causing unapproved overtime. Teamwork: Work cooperatively with others; value others’ contributions; be courteous; communicate openly; listen; show respect to others. Workplace Safety: Play a role in developing and maintaining a safe workplace; comply with all safety policies and procedures; report unsafe hazards to supervisor and participate in safety training. Maintenance Vehicles: Transport individuals into the community; drive safely and in accordance with local laws; use safety equipment such as seat belts, lifts, and wheelchair ties; report accidents to authorities and supervisor immediately. Housekeeping: Perform daily and seasonal housekeeping as directed; maintain a neat, clean, and safe environment; report hazards or safety concerns. Maintenance and Repair: Monitor environmental safety; perform minor repairs such as changing light bulbs; conduct yard maintenance including walkways and snow removal; report equipment or facilities needing repair to supervisor or maintenance personnel. Safety: Check water temperature when assisting with bathing; participate in safety drills; protect individuals in emergencies. Other Perform other duties and activities as required. MPA = Mobile Punch Authorized for timekeeping. Education and Experience High school diploma or equivalent preferred. Six months of experience in human services preferred. Must be at least 18 years of age. Working knowledge of computers. Certificates, Licenses, and Registrations Valid driver’s license in good standing. Car registration and vehicle insurance if providing transportation for individuals receiving services. All state‑required training(s) and certification(s) completed in mandated timeframes. Maintain valid driver’s license, meet insurance requirements, and be approved by management to be a driver. If an employee loses driving privileges due to MVR record violations, the employee must sign a Non‑Driver form and will be restricted from driving until the record meets company standards. Work Environment Work at the program location and may accompany individuals into the community. Physical Requirements Exert up to 100 pounds of force occasionally, up to 50 pounds frequently, or up to 20 pounds constantly to move objects. Additional Requirements Cooperate with licensee and Department of Human Services staff during inspections, inquiries, or investigations. Maintain a valid driver’s license in good standing in the state where the employee resides; if commuting from another state, maintain a valid license in the resident state. High school diploma or equivalent required. All employees hired after 6/15/2018 must submit to drug testing prior to employment; all employees will undergo periodic drug testing for cause, suspicion, or random screening. Submit to criminal background checks and avoid disqualification by the Department of Human Services; name will be checked against the Central Registry of Offenders Against Individuals with Developmental Disabilities and the Child Abuse Registry Information (CARI). Successfully complete and demonstrate proficiency in all required trainings. Must be at least 18 years of age.
