Promote the full integration and functioning of Persons Served so they can live healthy lives in the community. Provide direct service and routine nursing aide care under the direction of a Nurse in a clinically complex program serving medical and psychiatric needs. Assist with activities of daily living according to treatment plans and ISPs, including vital signs, laundry, toileting, showering, dressing, shaving, wound care, foot care, skin checks, and passive range-of-motion exercises. Monitor individuals for changes in physical, mental, or emotional status. Administer medications and treatments according to MAP guidelines and Vinfen policies. Provide nutritional support and training in ADLs, personal hygiene, dressing, and household chores. Conduct fire and safety training and document it as required. Provide transportation to medical, dental, and other appointments as directed. Maintain safe, clean, welcoming, and respectful living and common areas, including housekeeping, snow shoveling, and garbage removal. Document nursing care, medication administration, daily shift notes, incidents, medication errors, and maintain accurate confidential records. Communicate status changes and care information with managers, colleagues, families, guardians, and Vinfen staff. Safely use and maintain assistive and medical equipment, including wheelchairs, Hoyer lifts, walkers, glucometers, nebulizers, and oxygen. Support participation in home, program, and community events while respecting individual preferences and choices. Advocate for individuals served and maintain effective relationships with individuals, families, guardians, agencies, and community providers. Attend and participate in staff meetings, trainings, and community meetings. Act as a positive role model and assist with orientation of new staff. Adhere to Vinfen policies and procedures and perform other assigned duties. Requirements A high school diploma or equivalent is required. Associate's degree in a human service discipline preferred. One year as a residential counselor in a setting working with PS facing mental health challenges preferred. One to two years of job-specific experience in the human services field preferred. Preference given to bilingual/bicultural applicants and applicants with lived experience of psychiatric conditions. Skills and competencies of a C.N.A. Knowledge of brain injury. Ability to work effectively and collaboratively with colleagues. Ability to accurately and effectively communicate verbally. Ability to accurately and effectively document nursing care provided. Sensitivity to cultural, religious, ethnic, disability, and gender issues. Knowledge of human, legal, civil rights, community, and other resources. Knowledge of empowerment and self-advocacy techniques. Knowledge of teaching modalities. Ability to balance competing priorities. Ability to make independent judgments and decisions. Ability to work in a professional and confidential capacity. Knowledge of personal computer applications and equipment. Knowledge of documentation standards requirements. Valid driver's license required at employment; must maintain adequate insurance if using a personal vehicle and a safe driving record. Must be at least 21 years of age and have maintained a valid US driver's license for at least six months. Must pass a driver's screening background check. Ability to perform physically active duties, including standing, walking, bending, kneeling, stooping, crouching, crawling, climbing, and lifting at least 25 pounds. CPR required within two weeks of hire. First Aid required within two weeks of hire. MAP required within 150 days of hire.
