Care Coordinator- LPNSentara Health Plans Roanoke is looking to hire a Care Coordinator- LPN.Sentara Health Plans is hiring a remote Care Coordinator for the Roanoke/Lynchburg/Danville area!This position is remote work from home with travel to nursing facilities required.Hours of operation are M-F from 8:00am to 5:00 pm. Applicants must be in the Roanoke/Lynchburg/Danville area.The Care Coordinator performs a variety of casework duties and provides case management services to members in nursing facilities, including patients, families, and designated caregivers and requires travel to assigned facilities. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate.
