That means better support, opportunities for growth, and a workplace where team members can build a meaningful career in healthcare.What You’ll DoThe MDS Coordinator plays a key role in supporting quality resident care through accurate assessments, documentation, and individualized care planning. This position helps ensure regulatory compliance, appropriate services, and strong collaboration with the interdisciplinary team while promoting resident choice, dignity, and involvement in their care.Key ResponsibilitiesEnsure accurate, timely, and complete MDS assessments, including Admission, PPS, Quarterly, Annual, and Significant Change assessments, in compliance with RAI guidelines.Complete Medicare and Medicaid case-mix documentation to support appropriate reimbursement and help avoid default days or unassigned days.Work closely with the Director of Rehab and interdisciplinary team to select optimal ARDs and support effective care planning.Track and monitor Skilled and Managed Care residents to ensure continued eligibility, appropriate benefit utilization, and timely documentation.Gather and communicate information for Managed Care reviews, physician certifications, and re-certifications per Medicare requirements.Conduct regular audits of MDS coding, outcomes, validation reports, and supporting documentation to maintain accuracy and compliance with CMS policy.Submit MDS assessments electronically, review error and validation reports, and address late, missed, or rejected assessments as needed.Provide training and guidance to staff on the RAI process, MDS expectations, documentation requirements, and regulatory updates.Serve as a clinical reimbursement resource for nursing, therapy, business office, administration, and interdisciplinary team members.Actively participate in Case Management, Triple Check, Level of Care meetings, and support ADRs, audits, or additional documentation requests as needed.Maintain communication with administration, regional support, and the business office regarding case mix, reimbursement impact, and clinical documentation quality.Support resident-centered care planning by helping ensure resident preferences, care needs, and clinical changes are accurately reflected.Complete other duties as assigned or neededWhat We’re Looking ForActive Registered Nurse (RN) license in the state of Texas in good standing.Must have successfully completed an RN program at an accredited vocational school, college, or university.Minimum of two years of skilled nursing experience required.MDS, RAI, Medicare, Medicaid, case-mix, care planning, or clinical reimbursement experience preferred.Knowledge of MDS 3.0, RAI guidelines, PPS assessments, ARDs, care plans, and skilled nursing documentation preferred.Ability to review clinical documentation, identify gaps, communicate findings, and follow up with appropriate team members.Strong organization, attention to detail, time management, communication, and follow-up skills.Ability to work cooperatively with residents, families, nursing, therapy, business office, administration, physicians, and regional support teams.Must complete and pass all RIHS-specific MDS/RUGs training modules (AIS) within the first 90 days of employment and ongoing per company policy.Must be eligible to work in the United States.Why Work for Wellsential Health?Competitive payMajor medical, dental, and vision insuranceSupplemental insurance options including critical illness, hospital, accident, short-term disability, and life insuranceEmployee Assistance ProgramTuition Assistance ProgramPayActiv Paycard - access your paycheck up to 2 days faster, plus pay advance optionsEmpower 401(k)Holiday, Paid Time Off, and Bereavement benefitsTelehealth services with $0 copayShoes for Crews reimbursementApply today and join a team where your clinical knowledge, attention to detail, and commitment to resident care can make a real difference.Active Registered Nurse (RN) license in the state of Texas in good standing.Must have successfully completed an RN program at an accredited vocational school, college, or university.Minimum of two years of skilled nursing experience required.Complete and pass all RIHS specific MDS/RUGs training modules (AIS) within the first 90 days of employment and ongoing per company policy.MDS, RAI, Medicare, Medicaid, case-mix, care planning, or clinical reimbursement experience preferred.Knowledge of MDS 3.0, RAI guidelines, PPS assessments, ARDs, care plans, and skilled nursing documentation preferred.Ability to review clinical documentation, identify gaps, communicate findings, and follow up with appropriate team members.Strong organization, attention to detail, time management, communication, and follow-up skills.Ability to work cooperatively with residents, families, nursing, therapy, business office, administration, physicians, and regional support teams.Must complete and pass all RIHS-specific MDS/RUGs training modules (AIS) within the first 90 days of employment and ongoing per company policy.Must be eligible to work in the United States. #J-18808-Ljbffr Starr-County-Nursing-and-Transitional-Care
