Field-Based Northeast Ohio Care Coordination Specialist

CVS Health

CVS Health Corporation is seeking a Field-based Case Management Coordinator for Northeast Ohio. This full-time role requires traveling to member homes and other locations up to 50% of the time, primarily in counties like Cuyahoga, Lorain, Medina, Summit, Lake, and Geauga.

Work hours are Monday–Friday, 8 am–5 pm with flexibility to meet member needs. The role focuses on coordinating care for children with behavioral health needs within Ohio’s Medicaid program, collaborating with MCOs, vendors and
