Licensed Practical Nurse - Sign-On Bonus & Impactful Care

$5,000 SIGN-ON BONUS!!!JOB PURPOSE:Provides direct and indirect patient care services that meet the psychosocial, physical, and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a Registered Nurse. Any position that has direct patient care is considered to be safety sensitive by AVEM Health Partners standards.ESSENTIAL FUNCTIONS INCLUDE BUT NOT LIMITED TO:Responds quickly and accurately to changes in condition or response to treatment.Responsible for fostering a positive work environment.Assesses patients’ needs based on medical history, risk factors, complaint, need for care, etc. Classifies the urgency of symptoms and notifies appropriate personnel.Collects samples from patients including blood, urine, stool, and sputum. Prepares samples for analysis or shipment.Performs nursing care per qualifications, based on the plan of care, following established policies, procedures and/or protocol.Actively participates in the planning and implementation of patient care under the direction of the Registered Nurse.Delegates appropriately according to scope of practice and educational preparation; adhering to the Oklahoma Nursing Practice Act and “5 Rights of Delegation”.Administers medications as stipulated by the policies and procedures of the facility.Communicates with all members of the healthcare team.Ensures that required documentation is complete and in compliance with regulations and standards.Observes signs and symptoms and reports to the physician and/or other appropriate health professionals as often as needed, changes in the patient condition.Responsible for keeping current in nursing trends and evidence-based practice; participates in required in-service programs.Ensure all safety and infection control practices are followed.Respond to emergency situations.Maintain confidentiality of patient and organization.Perform other duties as assigned within scope of practice.BEHAVIORAL STANDARDS:The individual must support the mission, vision, and goals of Avem Health Partners and serve as a role model within the company.Exhibit positive customer service behavior in daily work interactions.Demonstrate a courteous and respectful attitude to internal workforce and external customers.Communicate accurately and appropriately.Handle difficult situations in a discreet and professional manner.Hold self-accountable for professional practice.Interact and cope effectively with diverse populations in stressful situations resulting from trauma and illness.Participate in performance improvement activities utilizing principles to support and improve patient care.Demonstrate knowledge of unit goals and participate in committees/projects to achieve these goals.Ongoing professional development regarding evidence-based practices and implementation of interventions to improve patient outcomes.Responsible for seeking out educational/professional opportunities for personal learning needs and growth and meeting mandatory education requirements.Act as a preceptor as requested.Demonstrate excellent work attendance and actively participate in a variety of meetings and training sessions as required.Adhere to the Avem Health Partners Code of Conduct and Standards of Behavior. Complies with established policies and procedures and all health and safety requirements.EDUCATION/CERTIFICATION/LICENSURE:Graduate of an approved Nursing Program.Current LPN license issued by the Oklahoma Board of Nursing.Current BLS (or obtained within 30 days of hire).Current ACLS preferred.EXPERIENCE:Two (2) years of clinical experience preferred.PHYSICAL REQUIREMENTS:To perform this job successfully, an individual must be able to perform each essential job duty satisfactorily.
