Position Description: Serve as a primary care provider for geriatric patients located in a skilled nursing facility. Overall duties of the role include, but are not limited to: plan, direct, and evaluate medical care examine and treat chronic illnesses examine and treat urgent illnesses order and interpret laboratory and diagnostic testing collaborate with caregivers and other healthcare professionals work closely with the interdisciplinary care team make referrals as necessary.

The practice uses an EHR on a mobile-based platform and providers are supported by team members who assist with scheduling, orders, referrals, etc. Providers share on-call responsibilities, which is low volume phone triage. Providers are on call no more than 3 nights per month and flexibility is offered.

Requirements: North Carolina NP license DEA 1+ year advanced practice experience 1+ year experience in a relevant setting
