Patient Information

Patient Demographics: Mostly all pediatrics, some adults with developmental delays and double vision

Average Daily Volume: 1214 patients per day

Case Mix: Bread and butter and high acuity peds optometry, strabismus/binocular vision

Job Requirements

Board: Pediatric optometry residency highly preferred

Licenses: CA license required

DEA: CA DEA with Schedule 2/2n required

Certifications: BLS (AHA)

Experience: Minimum 3 years' experience working with complex pediatric populations in a medical center setting

Submission Requirements

Presentations: Please include current CV, with gaps greater than 6 months explained, the provider questionnaire, and license verification.

Name Clear Process: The facility only requires name clears for providers who have previously or currently work with the facility system. Name clears will be sent to the appropriate contact who expects to clear within 72 hours.

Malpractice and disciplinary background issues: If your provider has more than 500K in malpractice or more than 2 cases please include first and third party documentation on the cases.

Credentialing/Privileging: Privileging generally takes 60-90 days. It's imperative that the provider is available to send their DOB, SSN, License, contact information, and a signed form sent during the offer process within 72 hours of signing. This allows for the facility to kick off privileging and any delays will impact start time. No emergency privileges.

Parking: The facility does not offer parking passes. Parking costs will need to be covered by the provider. There is public transportation available through the BART with shuttles going to each campus on a regular basis.

Duties & Responsibilities

Perform comprehensive pediatric optometry examinations and patient care.

Conduct work-ups and diagnostic imaging for high acuity pediatric patients.

Provide contact lens fitting and complex contact lens fitting for pediatric populations.

Co-manage patients with other pediatric specialties, including those with seizures and facial abnormalities.

Manage patients of all cognitive abilities, including those on the autism spectrum.
