The position travels locally throughout the greater Lebanon, Ohio area (Lebanon, Middletown, Springboro, Dayton and part of Cincinnati) providing patient services in a Home Health care setting. Shifts PRN Responsibilities Responsible for providing assistance with the primary caseload for our skilled home health care patients. Provide skilled professional nursing care to patients, their families and caregivers as prescribed by the physician and in compliance with the Home Health Medicare Conditions of Participation, Ohio Home Health licensure laws and agency policies and procedures. Qualifications Graduate of an approved nursing program. Valid Ohio Licensed Practical Nurse (LPN) license, CPR certification. Minimum of 2 years direct patient care experience in hospice or home care. Licensed driver with a clean driving record and automobile insurance in accordance with Ohio laws.
