Responsibilities Provide outpatient rehabilitation services in an in-home setting for older adults, either in private residences or independent living facilities. Assess, develop, and deliver physical therapy programs to facilitate rehabilitation, including making referrals to other disciplines. Implement individualized therapy programs to restore, reinforce, and enhance performance. Instruct, educate, and train patients and caregivers in essential skills and functions for independence and productivity. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel per state licensure requirements. Ensure adherence to treatment schedule, timely documentation, and participation in work-related meetings. Attend and contribute to patient care, staffing conferences, and other related meetings. Make referrals to other disciplines and agencies for comprehensive patient needs. Consult with rehabilitation team members to select appropriate therapeutic regimens. Adapt program and/or plan of care according to patient needs. Promote a safe environment and eliminate fire and safety hazards. Order, maintain, and report necessary supplies and equipment. Ensure patients and families receive the highest quality service in a caring and compassionate atmosphere. Screen, examine, and evaluate patients, including history, systems review, and appropriate tests and measures, to establish diagnosis, identify impairments, and design a plan of care. Provide therapy techniques such as: Therapeutic exercise (aerobic conditioning, balance coordination, agility, body mechanics, postural stabilization, flexibility, gait, neuromotor development, relaxation, strength, power, endurance) Functional training Manual therapy Airway clearance techniques Integumentary repair and protection Electrotherapy, physical agent and mechanical modalities Health and wellness programs Education on chronic disease management Falls assessment and interventions Case management in the home and community Design, fabricate, fit, and/or apply adaptive devices, and train patients in their use. Select activities suited to the individual's physical and mental capacity and interests. Instruct patients in mobility skills and evaluate progress, attitudes, and behavior related to rehabilitative potential. Perform routine re-examinations to modify or progress the treatment plan. Incorporate health and wellness programming into treatment plans. Perform home and community mobility assessments and recommend safe mobility solutions. Perform other related duties as required. Qualifications Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Master's degree in Physical Therapy, or a Master's in a related field plus a Bachelor's in Physical Therapy. Or a Bachelor's in Physical Therapy plus 5 years of progressively responsible PT experience. Internationally earned PT degrees evaluated as U.S. equivalents. Valid driver's license or reliable independent transportation to patients' residences. Compensation Salary range: $50.00 - $63.00 per hour. Bonus: $5,000.00. #J-18808-Ljbffr Powerback
