As a Wound Care Nurse Practitioner, you'll step into a role where your clinical decisions have measurable results. You'll work directly with patients across assisted living, memory care, skilled nursing, and post-acute communities while helping improve healing rates, reduce avoidable complications, and strengthen care coordination. What You’ll Be Doing Provide advanced wound care assessments, treatment planning, and ongoing management of complex wounds. Conduct routine rounding visits throughout post-acute, assisted living, memory care, and long-term care communities. Manage pressure injuries, surgical wounds, diabetic ulcers, skin integrity concerns, and related conditions. Collaborate closely with physicians, nurses, therapists, and facility teams. Support chronic disease management, preventative care initiatives, and continuity of care. Help reduce avoidable hospital readmissions through proactive intervention and follow-up. Qualifications Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred). Experience in wound care, post-acute care, geriatrics, primary care, skilled nursing, or related settings preferred. Strong clinical judgment and confidence managing medically complex patient populations. Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
