8-hour shifts Assignment Details
Contract Length: 13 weeks
Guaranteed Hours: 40 hours per week Requirements
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 1-2 years of experience as a Registered Nurse in a clinical care setting
- 2+ years of Home health experience
- Proof of current CPR certification
- Current driver's license
- Must have reliable transportation and insurance Roles & Responsibilities
- Assess patients in home health and/or hospice settings to identify their physical, psychosocial, and environmental needs.
- Implement and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and families on care, medications, and safety, promoting self-care practices.
- Monitor and assess pain, utilizing appropriate management techniques.
- Provide clinical direction to support staff to ensure quality service delivery.
- Communicate effectively with patients, families, team members, and other healthcare professionals. Compliance / Notes
- Must maintain confidentiality.
- Ability to work flexible hours and travel locally.
